The challenge

Observing the rising prevalence of prediabetes and other chronic diseases, PEHP sought new and innovative ways to help members understand their personal health risks and engage them in taking action to improve their health.

PEHP identified a priority population of health plan members—ages 45 and up—who hadn’t had a preventive visit in 3+ years. No labs. No check-ins. Little engagement. For PEHP’s wellness team, this was a red flag for rising risk and increasing costs.

In many cases, bringing those members into a clinic or on-site screening event simply wasn’t feasible—especially across rural regions of Utah.

They needed an easily-accessible way to reach this “invisible” population—and a way to make taking action feel easy, not overwhelming.

The solution

PEHP partnered with Reperio Health to launch at-home, instant-result preventive screenings with a segment of high-risk, low-engagement members. The approach was simple:

The pilot focused on members ages 45+ who hadn’t had a preventive visit in 3+ years—those most likely to be disengaged and overdue for a clinical touchpoint.

The results

→ 75 % activation rate

→ 85 % completion rate

In the world of preventive healthcare—where engagement is notoriously low—seeing a 75% activation rate and an 85% completion rate is exceptional. These numbers reflect a highly accessible, member-centered experience that overcomes the usual barriers that stall early detection and preventive action.

Even without digital ads or complex automation, engagement soared—thanks to a simple direct mailer campaign.

PEHP’s decision to send ~1,200 mailers led to significantly higher participation than their internal email system. Once members received their kits, the response was overwhelmingly positive:

→ CSAT: 88.9%

Completion wasn’t limited to one region either. Of the 156 members with location data:

→ 53% lived in urban areas

→ 44% lived in rural areas

This cross-section demonstrated that the pilot project worked across the geographic spectrum.

Deeper impact


The at-home screenings were just the beginning of the "Healthy Utah at Home" pilot project.

PEHP immediately moved to activate those who had completed their kits. The wellness team began inviting members to schedule follow-up appointments with internal biometric screeners—an opportunity to go over their results, access resources, and connect with a provider if needed.

These 1:1 follow-ups are still in motion—but the shift is clear:
